You may download a form to release records from a previous health care provider to us at Garrison Women's Health Center. Please send this release to your former provider.
You may also download a form to release information in Garrison's or New England Mammography's chart to yourself or another provider. Please return this form to Attn: Medical Records Department at Garrison Women's Health Center.
